Lead Personalized Recovery Pathways: Design and monitor
comprehensive care plans that empower patients to regain their
independence and reach their specific physical milestones. -
Synchronize the Multi-Specialty Team: Act as the primary clinical
hub between physical, occupational, and speech therapists to ensure
a unified and seamless approach to the patient's rehabilitation. -
Execute Precise Clinical Care: Carry out physician and nursing
directives with a focus on safety and progress, ensuring every
treatment supports the patient’s overall stability and growth. -
Architect Successful Home Transitions: Evaluate and coordinate
complex discharge needs early in the process, ensuring patients and
their families have the resources and confidence to transition
safely back to their community. What minimum requirements you’ll
need Licensure / Certification / Registration: - BLS Provider
obtained within 1 Month (30 days) of hire date or job transfer date
required. American Heart Association or American Red Cross
accepted. - Registered Nurse credentialed from the Indiana Board of
Nursing obtained prior to hire date or job transfer date required.
Education: - Diploma from an accredited school/college of nursing
OR Required professional licensure at time of hire required. Equal
