How you’ll make an impact in this role - Orchestrate Comprehensive Care Plans: Lead the implementation and continuous monitoring of personalized care strategies to ensure every patient reaches their recovery milestones. - Champion Multi-Disciplinary Coordination: Act as the central point of contact for the entire care team, synchronizing specialists and support staff to provide a seamless, holistic patient experience. - Translate Clinical Orders into Action: Execute physician and nursing directives with precision, ensuring that treatments are delivered safely and effectively. - Facilitate Safe Transitions: Evaluate and coordinate discharge needs early in the stay, working closely with the healthcare team to ensure patients have everything they need for a successful return home. What minimum requirements you’ll need Licensure / Certification / Registration: - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. - Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Education: - Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
