Responsibilities Conduct comprehensive health assessments for
adults and geriatric patients, including medical history,
diagnosis, treatment, health education, and referrals for case
management and follow-up. Administer and order necessary
point-of-care and lab-based tests. Collaborate with physicians,
case managers, social workers, family members, and other medical
personnel as needed. Work closely with Primary Care Physicians
(PCPs) to provide patient education and follow-up care. Deliver
services at the member's residence, which may include private homes
or skilled nursing facilities. Conduct assessments from Sunday to
Saturday, between 8 AM and 7 PM, with flexible scheduling options
for day, evening, and weekend shifts. Enhance your earning

outside your home location. Requirements and Qualifications AANP,
AACN, or ANCC board certification in one of the following
specialties: Adult, Adult Gerontology Primary Care, Adult
Gerontology Acute Care, Family, Emergency, or Acute Care.
Approximately one year of Nurse Practitioner experience is
preferred; however, new graduates are encouraged to apply!
Proficient computer skills and familiarity with health/medical
record software. Working Conditions We provide comprehensive
onboarding training, assign a preceptor for guidance, and offer
one-on-one feedback. Additionally, you will have free access to
continuing education opportunities. We support you in obtaining
additional state licensure and credentialing in neighboring states
or other locations you may wish to visit while conducting health
