Job Description: Essential Duties and Responsibilities include
the following. Other duties may be assigned. 1. Demonstrates fiscal

Accountability Weight 1. Provides complete verbal report with
appropriate information. Effectively communicates to other care
providers/team members necessary information. Delegate tasks based
on patient need and staff competency, and assure tasks are
completed. Responds appropriately to customer requests. Transcribes
or checks transcription of physician orders.2. Performs admission
assessment and history taking, recognizing the individuals
physical, emotional and educational needs. Performs ongoing
physical assessment for purpose of evaluation of patient and
patients response to care. Performs ongoing psychosocial and
spiritual evaluation for purpose of identifying nursing diagnosis
and making referrals.3. Formulates/Implements Nursing Plan of Care.
Selects and implements nursing interventions based on assessment
and nursing diagnosis. Prioritizes nursing plan of care on
admission based on patients identified needs. Develops mutual goals
with patient and family. Completes nursing plan of care/discharge
plan in computer system4. Makes appropriate decisions utilizing
evidence based practice. Recognizes changes in patients physical
and emotional condition and institutes interventions. Makes
decisions in crisis situations based on assessment and prompt
intervention. Assumes and is accountable for delegation and
supervising total patient care. Establishes realistic workflow
priorities delegating appropriate tasks to assure work is completed
within allotted time5. Initiates and discontinues IV therapy as
directed. Changes IV bags and tubing and labels appropriately.
Monitors level of IVF and observes site per policy. Demonstrates
knowledge and ability to utilize IV infusion device as applicable.
Administers and documents IV fluids and blood/blood products6.
Demonstrates knowledge of expected outcome and side effects of all
drugs administered. Demonstrates knowledge of special
administration techniques (IM, Z-tract, Sub-Q, IV push). Documents
patient response to pain medications. Counts and documents
narcotics and follows through with problems. Documents medications
administered per hospital policy and procedure. Successful
completion of monthly medication newsletters.7. Supervises the
proper care, use and repair of supplies, equipment and charges.
Checks and documents emergency equipment preparedness per policy.
Keeps patient room and hallways free of clutter. Disposes of soiled
linen in appropriate container. Keeps nursing station clean and
neat8. Performs professional responsibilities9. Acts as a
professional role model at all times10. Functions as mentor for new
employees11. Functions as team leader, team member or primary care
giver performing patient care and various unit activities12. Makes
decisions and adapts to unit changes precipitated by admissions and
increased workloads13. Demonstrates ability to address staff/peers
with concerns related to performance expectations and behaviors
that may effect work environment, image, patient care and customer
service14. Requests and accepts constructive criticism15.
Accountable for accuracy and completion of all documentation
requirements16. Documents all care given, including patient
preparation and patient response to treatment17. Documents
physical/emotional care rendered with patient response and/or
outcome18. Provides effective care in a code or crisis situation19.
Acts as a preceptor when applicable, identifying and following
through with performance or knowledge deficits20. Communicates with
others in completing work assignments and accomplishing
department/unit goals. Utilizes appropriate channels of
communication21. Monitors/ transcribes HEO for Physician entered
orders. Keeps physician thoroughly informed of any sudden changes
in patients condition or insidious changes requiring medical
intervention. Consults ancillary services (Case Management, Social
Services, etc) for evaluation and/or assistance. Communicates
appropriate information to outside related agencies (policy, health
department, etc.). Completes written reports with appropriate
information22. Assess patient/family/SO interactions that may
impact patient care. Recognizes pertinent and individual patient
needs. Reevaluates on an ongoing basis nursing care based on
progress towards goal, patient outcomes, and updates plan of care
as necessary23. Performs and reinforces formal and informal patient
teaching, utilizing available resources. Reinforces patient
teaching, including teaching done by physician and other
departments.24. Successful completion of Age Specific competencies
for the gerontology, adult, adolescent and pediatric patient.25.
Provides care/to the ventilator dependent patient26. Monitors EKG,
invasive lines, hemo-dynamics and respiratory parameters of the
critical care patient27. Biannual EKG and Sedation required28.
Annual nursing, hospital wide and unit specific skills (which
include Femstop and IABP)29. Maintains up to date BLS
certification30. ACLS certification preferred within 3 months, not
to exceed 6 months from date of hire.If you would like to be part
