Madisonville, Health and Rehabilitation is currently hiring for a
full-time position that will play a dual role sharing job functions
as part-time SDC 20 hours per week and a floor nurse 20 hours
weekly for our skilled nursing facility! Come join our team at MHR

Qualifications & Educations:
RN Licensure within the State plus a minimum
Requirements:
Previous experience working with geriatric population is
preferred
Must be a licensed RN nurse.
Must have experience in supervision and principles, practice of
teaching, etc.
Experience in rehabilitative and restorative nursing practices
strongly preferred.
Must demonstrate the knowledge and skills necessary to provide care
appropriate to the age-related needs of the residents served.
Must be a supportive team member, contribute to and be an example
of teamwork and team concept.
Strong communication skills with the ability to deal tactfully with
personnel, residents, family members, visitors, government
agencies/personnel, and the public.
Responsibilities:
Works in collaboration with facility team to assess the needs of
the resident.
Assists with coordination and management of care management
meetings, to include review of resident care and the setting of
assessment reference dates.
Provide support to the Director of Nursing with planning,
developing, organizing, implementing, and evaluating the nursing
service department, as well as its programs and activities, in
accordance with current rules, regulations, and guidelines that
govern the nursing care facilities.
Develop methods for coordination of nursing services with other
resident services to ensure the continuity of the residents' total
regimen of care.
Review nurses' notes to ensure that they are informative and
descriptive of the nursing care being provided, that they reflect
the resident's response to the care, and that such care is provided
in accordance with the resident's wishes.
Incorporate commercially produced instruction material and training
aids into existing in-service programs as deemed necessary.
Provide leadership in formulating the goals and objectives of the
in-service educational programs of this Center.
Responsible for Quality Assurance Performance Improvement.
Ensure all departments are monitoring their performance indicators
and lead all performance improvement projects. Perform additional
duties as assigned.
