Job Description:
Performs as a member of the health care team to assist the
professional nurse in meeting the goal of individualized care by
performing designated elements of patient care and/or unit
activities. Holds IV certification that meets Kentucky Board of
nursing criteria.
Essential Duties and Responsibilities include the following. Other
duties may be assigned.

Provides complete verbal report with appropriate information.
Effectively communicates to other care providers'/team member's
necessary information. Delegates tasks based on patient need and
staff competency, and assures tasks are completed. Responds

Observation (Assessment-in participation with the registered
nurse). Performs ongoing physical observation for purpose of
evaluation of patient and patient's response to care. Performs
ongoing psychosocial and spiritual observation for purpose of
evaluation of patient and patient's response to care.
3.
Development/Modification of Plan of Care. Modifies (updates) the
plan of care based on observation of the patient's status. Develops
mutual goals with patient and family. Completes nursing plan of
care/discharge plan in computer system.
4.
Provides patient care. Bathing patient. Assists patient in range of
motion exercises (passive or active). Performs morning, afternoon
and evening care. Positions, turns and ambulates patient. Assists
patient with mobility needs. Changes bed linens of occupied and
unoccupied beds. Initiates and follows High Risk Fall Precautions
as indicated. Follows Suicide Precautions when applicable.
5.
Initiates and discontinues IV therapy as directed. Changes IV bags
and tubing and labels appropriately. Monitors level of IVF and
observes site. Documents medications administered per hospital
policy and procedure.
6.
An LPN shall perform only those IV therapy acts for which the LPN
possesses the knowledge, skill, and ability to perform in a safe
manner, except as limited by Section 6 of this administrative
regulation and under supervision as required by Section 3 of this
administrative regulation.
7.
An LPN shall obtain instruction and supervision as necessary if
implementing new or unfamiliar nursing practices or procedures. An
LPN shall follow the written, established policies and procedures

An LPN shall consult with an RN or other appropriate individual and
seek guidance as needed if: The patient's care needs exceed the
licensed practical nursing scope of practice; The patient's care
needs surpass the LPN's knowledge, skill, or ability; or The
patient's condition becomes unstable or imminent assistance is
needed.
9.
Performs the proper care, use and repair of supplies, equipment and
charges. Keeps patient room and hallways free of clutter. Disposes
of soiled linen in appropriate container. Keeps nursing station
clean and neat. Cleans equipment per policy and stores in
appropriate area.
10.
Performs professional responsibilities. Acts as a professional role
model at all times. Functions as team leader, team member or
primary care giver performing patient care and various unit
activities. Makes decisions and adapts to unit changes precipitated
by admissions and increased workloads. Verifies all orders via
computer system. Demonstrates fiscal responsibility through
appropriate utilization of resources. Reinforces patient and family
education throughout continuum of care.
11.
Accountable for accuracy and completion of all documentation
requirements. Documents all care given, including patient
preparation and patient response to treatment. Documents
physical/emotional care rendered with patient response and/or
outcome.
12.
Observes skin color, temp, turgor, peripheral pulses, pain, signs
and symptoms, mental status, LOC, respiratory rate and depth,
change of condition, edema etc. Observes patient therapy modes:
invasive or indwelling lines and tubes, i.e. Foley, NG, IV, drains,
etc.
13.
Performs catheter care. Performs perineal care. Uses lifts for
patient transfers when indicated. Takes vital signs, including
VAS#, and records in electronic record .
14.
Performs pulse oximetry as directed. Performs oropharyngeal
suctioning as indicated. Performs nasopharyngeal suctioning.
Performs nasotracheal suctioning. Performs tracheotomy
suctioning
15.
Transcribes and/or checks physician's orders. Keeps physician
thoroughly informed of any sudden changes in patient's condition or
insidious changes requiring medical intervention. Successful order
entry into computer system
16.
Communicates with others in completing work assignments and
accomplishing department/unit goals. Utilizes appropriate channels
of communication
17.
Evaluates patient/family/SO interactions that may impact patient
care. Recognizes pertinent and individual patient needs
18.
Performs Finger Stick Blood Sugars. Provides special skin care.
Prepares patients for physical exam and assists when directed.
Performs sterile dressing changes.
19.
Makes observations hourly. Frequency may increase dependent upon
patient status and unit standards. Measures and records weights.
Clears IV pump. Empties drains, catheters, drainage tubes.
Calculates intake and output. Collects, labels and routes
specimens
20.
Assists patient with incentive spirometry. Reinforces patient to
turn, cough, and deep breath. Monitors oxygen per protocol. Feeds
patients as indicated. Provides nourishments as directed. Performs
tube feedings via nasogastric tubes, Dobb Hoff tubes, changing
feedings, bag and tubing per policy. Passes fresh water as
needed
21.
Assists patient with elimination needs. Prepares and administers
enemas. Incontinence care. Performs care of new and established
colostomies and ileostomies. Inserts and removes Foley. Application
and monitoring of restrictive devices. Inserts rectal tubes.
22.
Demonstrates knowledge and ability to utilize IV infusion device.
Demonstrates knowledge of expected outcome and side effects of all
drugs administered. Demonstrates knowledge of special
administration techniques (IM, Z-tract, and Sub-Q).

Adheres to unit dress code
24.
Attendance at mandatory in-services, unit meetings, is maintained
in accordance with department guidelines
25.
Demonstrates ability to address staff/peers with concerns related
to performance expectations and behaviors that may effect work
environment, image, patient care and customer service. Requests and
accepts constructive criticism
26.
Maintains up to date annual Core and Nursing Department/Unit
Specific competencies and Certifications.
Minimum Education, Experience, Training, and Licensures
Required:
Graduate from an accredited School of Nursing required. Current
licensure as a Licensed Practical Nurse by the Kentucky Board of
Nursing. BCLS required. EXPERIENCE: No experience required for part
time and full time employees.
