SUMMARY OF THE JOB This is a Registered Nurse who coordinates patient throughput from all points of entry to facilitate overall hospital patient flow. This person works with a team in receiving all communication related to patients requiring a bed for appropriate clinical expertise and facility placement to support delivery of quality care. # The Patient Placement RN Lead collaborates with hospital staff, ADON, physicians, staff, resource team, and transfer center to define, reduce and eliminate barriers within the system which may impede patient flow and surge plan. This is will also coordinate nursing home triage calls as scheduling allows.# # # ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Job Duties include the following, other duties may be assigned: DEMONSTRATES CLINICAL COMPETENCE Provides effective communication with team members and other departments to ensure seamless process. Provides accurate and timely follow up with staff, physicians, patients, families and receiving facility/departments. Collaborates with hospital staff, physicians and their staff to define, reduce and eliminate barriers within the system which may impede patient flow. Applies knowledge of hospital and unit admission criteria and type of care provided in each department to appropriately place patients using critical thinking skills. Demonstrates competence in current knowledge of specific unit placement criteria May be required to fill in other call center functions. Evaluates and negotiates appropriate patient placement, prioritizing according to current census, staffing situations and knowledge of patient acuity. Responsible for obtaining proper information from physicians, physician office staff, and outside facilities for placement and admission assessment. Obtains and enters admission orders when appropriate. Documents and reviews pertinent clinical information for appropriate placement. Coordinates interdepartmental transfers and discharges. Collaborates with ADON to problem solve issues related to admissions, transfers and discharges. Receives all physician office calls for direct admissions and outside facilities. Will keep in constant communication with office staff to facilitate the direct admission process. Analyzes performance of patient throughput systems and protocols using critical thinking skills. Works on departmental performance improvement projects as assigned. Supports facility performance improvement activities as assigned i.e. completing audits, data collection, etc. Demonstrates computer and systems competency in the following areas:# enter data; retrieves data; assembles and organizes data; uses and analyzes data; integrates diverse sources of data; electronic medical record (EMR). The education/experience, certifications, skills, physical demands, and work environment described below further define requirements associated with this position. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.# Education and Experience Graduate from an accredited school of professional nursing.# BSN preferred. # Minimum of 3-5 years clinical nursing experience in an acute care setting or related specialty area preferred. Certificates, Licenses, Registrations Current RN licensure in the state of Indiana Language Skills Ability to read documents and give instructions from a computer screen or manually.# Must possess excellent written, verbal and interpersonal communication skills.# Must demonstrate excellent listening skills.# Must also have excellent telephone skills and basic counseling skills.# Mathematical Skills Able to add, subtract, multiply # divide in all unit of measure, using whole numbers, common fractions # decimals.### Reasoning Ability Demonstrates exemplary nursing process skills and prioritization of symptoms described by the caller.# Ability to define the problem/situation, collect accurate and adequate data, utilize appropriate guideline and draw valid conclusions.# Must exhibit excellent time management and prioritization skills. Computer Skills Basic personal computer skills, keyboarding, and comfort with Window operating systems.# Includes all of the following skills:# Enters data, retrieves data, assembles and organizes data, uses and analyzes data and integrates diverse sources of data and systems.# Able to prioritize and manage multiple applications.# Ability to troubleshoot and problem solve with the systems within the Call Center and seek assistance for resolution as necessary.
