Job Description:
The SASS Case Manager works to alleviate or stabilize mental
health symptoms of selected clients, which impairs their ability to
adjust to home, community integration, or maintain living
stability; provides therapeutic services to individuals, families
or groups regarding psychological or emotional problems such as
therapeutic settings by performing the following duties; develops
assessment and treatment planning for
clients.QualificationsApplicants must meet the requirement for a
Mental Health Professional as defined by Rule 132. This
includes:Therapist - candidates who have a master's degree in
counseling and guidance, rehabilitation counseling, social work,
vocational counseling, psychology, pastoral counseling, or family
therapy, or a related fieldCase Manager - Candidates who have a
bachelor's degree in a human services related fieldScreening
Assessment and Support Services (SASS)Screening Assessment and
Support Services (SASS) is an intensive mental health program
designed to provide crisis stabilization, pre-admission screening,
assessment, and treatment of any child who may be at risk of
psychiatric hospitalization and who is eligible for public funding
under any program funded by one of three collaborating departments
(IDHS, IHFS, and IDCFS). Services are child-centered,
family-focused, community-based, and culturally competent, and
occurring in the least restrictive setting, and include
multi-systems.Services OfferedSASS provides 24 hours a day, 365
days a year screening, assessment, and follow up services,
including case management, linkage, community support, and
counseling/therapy, to youth referred to One Hope United by the
Crisis and Referral Entry Services (CARES) Hotline and any youth
residing within the counties of the Illinois Local Area Networks
(LAN) of Washington County in LAN 7, Jefferson, Hamilton, or Wayne
Counties in LAN 8, or Jasper, Clay, Marion, Fayette, and Effingham
Counties within LAN 9. Transfer cases from other SASS programs are
accepted, as well.Job Duties Include:Must be energetic, a team
player, and have strong written and communication skills. Basic
computer skills required. Duties includeDaytime crisis intervention
services in Hamilton, Jefferson, Wayne, and Marion counties.Ensure
the safety of youthIndividual and family crisis intervention and
counselingConduct Crisis/Safety AssessmentDevelop and implement
crisis stabilization or deflection plansProvide 90 days of
follow-up therapy and case management servicesComplete Mental
Health Assessments, Treatment Plans, and other Case Management
servicesComplete required paperwork within established
time-linesProvides comprehensive and initial diagnostic assessment
of all referred clients utilizing interviews and well-recognized
criteriaDevelops treatment plans using a variety of generally
accepted intervention techniques to address areas of assessed
needPlans and administers therapeutic treatmentMaintains
comprehensive client file with progress/process notes, completing
assessments and quarterly reports, closing summaries, preparing
reports for court as needed, testifying in court as needed, and
participating in multidisciplinary staffing as neededWe Invest In
